Shift: 7:00 AM - 3:00 PM
Payrate: $35 - $40/hr (Depending on experience)

Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a per diem (PRN) position at a skilled nursing facility in Gloucester, MA . Key Responsibilities:
- Deliver direct patient care aligned with individualized care plans
- Administer medications and carry out physician-ordered treatments
- Monitor and document changes in patient conditions
- Support patient admissions, transfers, and discharges
- Accurately document care in the electronic medical record (EMR) system
- Collaborate with the nursing and interdisciplinary team to meet resident care goals
- Maintain a clean, safe, and supportive environment for residents
Required Requirements:
- Active and Valid LPN license in the state of Massachusetts.
- CPR/BLS Certification .
- Minimum 1 year of recent experience in a LTC/SNF setting.
- Strong clinical and communication skills.
